2. SAC Credential Supervisor Letter of Agreement
3. SAC Credential Personal Letter of Agreement

Return these completed forms, along with the $50.00 non-refundable Registration Fee to:

Registration is taken on a first-come, first-served basis. You will be notified of your acceptance within a month of the start of the next class. Please see SAC Class Schedule for the next available class.

A $100.00 books and materials fee is due the first day of class. Tuition costs may possibly be covered by the Educational Incentive Program (EIP). If you do not qualify for EIP, you will be financially responsible for tuition.
